Physical FormsNew Jersey State Code 6A:16-2.2 states that "student medical examinations shall be conducted at the medical home of the student and a full report sent to the school." It is our district's policy to encourage parents of students' who will enter grade three in September to obtain a physical examination and send in the documentation to school.
Your grade two student will bring home a physical examination form home this week. If you have any questions or concerns please contact me at school. Thank you. Health History UpdateIn order to keep your child's health history current, a health history update will be copming home with your Kindergarten, First and Third grade student this week. Please fill out and return to the health office in September. Thank you.
Kindergarten PhysicalsNew Jersey Administrative Code6A:16-2.2 requires parents to provide examination documentation of each student within 30 days upon enrolling in school. A physical form was included in the registration packet for parents who registered their child for Kindergarten starting in September. Please send in the documentation to school as soon as possible .

Last day of school for 2010-2011 Enjoy your Summer Help control the spead of Head lice in our schoolHead lice can be difficult to detect in a child's hair. If you see live lice in the hair you know that your child has lice. Live lice are tiny and wingless and move away quickly when you part the hair. You are more likely to see the eggs of the head lice. These eggs are glued to the shaft of the hair. They are oval and opaque and do not blow away like dandruff does. An itchy scalp can also be a sign of head lice but a child can have lice for a long time without their scalp becoming itchy. Prevention is the key in controlling an outbreak of head lice. Check your child every night. If you see nits ask the pharmacist or your physician about products that can be used to treat the outbreak. Follow the product directions exactly and do not over treat. Head lice are spread through direct head to head contact only. This is because the head louse cannot survive for long periods of time off the human scalp. Teach your children to brush and comb their hair every night. When checking your child's head be sure to check around the ears and the neck area. Be especially watchful after group activities such as sleepovers, scouting activities (Brownies, Cub Scouts), gymnastics, church outings, drama/ dance classes and camps where head to head or close personal contact could have occurred. If you keep close observation of your child’s head, you can detect the lice early before they have a chance to spread. If you find that your child has a case of head lice please contact the school nurses as soon as possible. Your child's class will be screened for any other cases. If you suspect that your child has head lice but are not sure, please bring your child to the nurses office before school starts and we will be happy to help you determine if there is a case present. The card covers routine dental procedures, orthodontics for children and adults and dental specialties such as endodontics, oral surgery, orthodontics and periodontics. There are 400 local providers. Dental program rates are: Individual $6.95 per month or $59.00 annually and Family $8.95 per month or $69.00 annually.

Remember to inform your child's school nurse before you go to any well child exams, or prior to a physician's visit when ever possible. Please ask the school nurse to provide you with a blank physical exam form and check with the school nurse to make sure your child is receiving the State of New Jersey mandated immunizations.

We strongly recommend annual physical examinations with your child's family health provider. Due to fees being charged by area physician's to complete multiple school forms, the NCS-MS is currently using only ONE standardized physical exam form, adopted by the State to be used for athletic participation. This physical form is quite comprehensive and provides valuable insight into your child's health status when completed correctly. Please contact the school nurse for a blank physical exam form prior to your child's scheduled annual physical exam or you may download a copy of this form from the state of New Jersey's web site at: www.state.nj.us/education/students/safety/health/records/athleticphysicalsform What about physical exams for sports and why are they done this way?N.J.A.C. 6A:16-2.2 (Required Student Medical Examinations) is an administrative code established by the NJ Department of Education.
